Thank you for considering the TMD All Faiths Chapel for your Special Event. We will do our best to honor your request.

Before filling out this questionnaire, please read the " Chapel Guidelines" before making your request.

Chapel Guidelines

Dressing Space: The Chapel does not have adequate dressing room space for events. It is best to come dressed and prepared prior to the commencement of your event.

Event Rehearsals: If request is granted, the Requestor is responsible for returning the Chapel to its original appearance. It is best practice to set up / decorate the Chapel during your scheduled rehearsal time.

Chapel Audio/Video Use: You are welcome to use the audio / visual capabilities of the Chapel! The Requestor is responsible for providing one individual to run the AV equipment. Please bring a flash-drive with music / video presentations or be prepared to Bluetooth sync your music selections to our equipment. Please test all AV presentations during your scheduled rehearsal day to ensure that everything works. 

Cancellations: The Office of the State Chaplain reserves the right to cancel any scheduled event due to mission requirements and Federal, State, or local emergencies.

Chapel Restoration & Cleanup: The Requestor is responsible for Chapel restoration and clean up to include returning the Chapel to its original appearance, removing all trash, and light vacuuming.

Items Not Permitted: Food or Drinks in the Chapel Sanctuary (covered water containers allowed), wax candles, live flower petals strewn on the carpet (silk flowers permitted), items with glitter, videos or music depicting nudity, risqué behavior, offensive language, misogyny-misandry, or racism. 

Note: The TMD Office of the State Chaplain is not responsible for personal items / unit items left in the Chapel before, during, or after the event.
